Designed with GPs and practice managers

We've digitised SIRA's NSW Certificate of Capacity/Fitness for Work into a smart form, built into HealthLink Forms, within your PMS, connecting directly to the patient record in front of you. Co-designed with GPs and practice managers and refined over three years of real-world feedback.

A faster, simpler way to complete the certificate

Time-saving

Pre-filled patient details and guided prompts mean less time per certificate, completed and submitted directly within your PMS.

Secure and Compliant

Enjoy peace of mind with encrypted submissions and automatic patient record updates.

Improved Patient Experience

Patients can sign their consent by QR code, email or SMS, wherever they are, including telehealth, so care isn’t held up by paperwork.

Why it's different

What makes our solution better?

GPs and practice managers helped shape the initial design, creating a solution focused on saving time, reducing manual effort, and fitting seamlessly into existing clinical workflows.

Digital NSW Certificate of Capacity FAQs

Who is Konnect NET?

Konnect NET is part of Lanas and specialises in connecting insurers with health providers. You may already know HealthLink, also part of Lanas, whose technology helps power the solution and makes it accessible to their large provider network.

How do I access the form?

Find the NSW Certificate of Capacity under the Spotlight Services section of HealthLink Forms also known as the HealthLink Homepage, in your practice software. If you cannot see it there, then you should find it in the Referral Services Section.

See the user guide above for more information.

Do I still need to sign the form?

No. It’s signed electronically based on your PMS login details.

Why am I asked to specify an insurer?

Specifying the insurer allows us to send the completed form directly to them (if they are participating in our solution), reducing delays and improving the patient experience. You have the option to tick the “Unknown” checkbox if you are unsure who the insurer is.

How do I send this form to the insurer?

Once you have completed the form, you simply hit ‘Complete’ and the form will be encrypted and sent securely to the insurer directly where relevant. In some cases you will need to give a copy to the patient for them to share the form with the insurer; in these cases, an onscreen prompt will help you do this.

Do I need to download a copy of the form?

On successful form completion, a PDF copy of the form will be automatically stored against the patient record. Depending on your practice software, you will find this under Correspondence Out or Letters. However, you have the option to download a copy manually also, by clicking the ‘Download/Print’ button. You may wish to do this to share a copy with your patient.

I have feedback, what should I do?

We’d love to hear it. Share suggestions using the feedback tool within the digital form, or contact our product team directly — see Support below.
